For the second year in a row, VNS Health Total has earned the highest possible 5-star Overall Quality Rating from the New York State Department of Health (NYSDOH), ranking it as New York’s top Medicaid Advantage Plus (MAP) plan.
The rating makes VNS Health Total the only MAP plan in the state to achieve five stars, based on NYSDOH’s evaluation of care quality and member satisfaction.
NYSDOH 5-Star Ratings Highlight Care Quality and Experience
NYSDOH’s latest consumer-facing ratings also awarded five stars to VNS Health’s Medicaid Managed Long Term Care (MLTC) plan, signaling strong performance in areas such as care coordination, access, and member experience.
Together, the ratings underscore VNS Health’s continued focus on serving people with complex medical and long-term care needs through integrated, person-centered models of care.
Integrated Medicare and Medicaid Coverage for Dual-Eligible Members
VNS Health Total (HMO D-SNP) serves individuals eligible for both Medicare and Medicaid who require support with activities of daily living. By combining Medicare Advantage and Medicaid Managed Long Term Care benefits into a single plan, VNS Health Total simplifies care delivery while supporting members’ ability to live safely at home.
Approximately 8,500 members are enrolled in VNS Health Total. Overall, VNS Health Health Plans serve more than 85,000 members statewide.

According to VNS Health leadership, earning five stars for a second consecutive year reflects long-term investment in care quality rather than short-term gains.

Serving Communities Across New York State
As of January 2025, VNS Health’s Medicaid MLTC plan is available in 38 New York State counties. VNS Health Medicare plans, including VNS Health Total, are offered throughout New York City’s five boroughs as well as Westchester, Nassau, Suffolk, Albany, Schenectady, Erie, Monroe, and Rensselaer counties.
