VNS Health offers specialized behavioral health services designed to support individuals with mental health or substance use disorders. If someone you know needs additional support, you can refer them to one of our community programs.
VNS Health can provide support to those living with:
- Addiction and substance use disorders
- Alzheimer’s disease or another type of dementia
- Anxiety disorders
- Bipolar disorder
- Major depressive disorders
- Other psychiatric diagnoses
Community Programs and Eligibility Requirements
VNS Health offers many community programs that might benefit your patients. Each program serves different people and different areas of New York. For information about these programs or to refer someone, please complete the Behavioral Health Referral Form.
Programs for Adults of All Ages
|Age 21+||Comprehensive care coordination integrating medical and behavioral health service needs for those with behavioral health and/or substance use problems, HIV/AIDS, and/or multiple chronic illnesses. Active Medicaid required.|
|Adult Mobile Crisis Teams*||Bronx Brooklyn|
|Age 21+||Short-term, in-home mental health services for people who are experiencing — or are at risk of experiencing — a mental health crisis.|
Programs for Older Adults
|Geriatric Mental Health Treatment Services||Bronx||Age 55+||Free in-home therapy for older adults to address depression provided by funding from New York City Council.|
|Geriatric Community Outreach||Bronx|
|Assessment, supportive counseling, and linkage to community services for older adults.|
|Age 55+||In-home treatment for homebound seniors that targets symptoms of depression by using intervention based on cognitive behavioral therapy.|
Programs for Children and Families
|Children and Family Support Team||Bronx||Age 5–17||Comprehensive assessment and in-home crisis intervention for seriously emotionally and behaviorally disturbed children and their families.|
|Children’s Health Home||Bronx|
|Age 6–21||Comprehensive care coordination integrating medical and behavioral health service needs for those with behavioral health and/or multiple chronic illnesses. Active Medicaid required.|
|Children’s Mobile Crisis Teams*||Bronx Brooklyn Queens||Age 0–20||24/7 rapid crisis care for children and teens and their families.|
|Outpatient Mental Health Clinic||Bronx||Age 5+||Assessment, treatment, and medication management for seriously emotionally and behaviorally disturbed children and their families. Insurance required.|
|Home-Based Crisis Intervention||Bronx Brooklyn||Age 5–18||Short-term services for families with children who are at risk of hospitalization due to serious mental health needs.|
Behavioral Health Support at Home
If you are a social worker, community partner, or social service partner, you might notice harmful behaviors in your clients. VNS Health’s behavioral health services can help individuals in all aspects of their lives — whether that’s building relationships, achieving their dreams, or completing daily tasks.
In addition to our community programs, VNS Health supports individuals with various home care plans. When needed, a behavioral health specialist is available to support home care nurses as a part of an individual’s overall care plan. This can be beneficial in helping someone address all aspects of their health — including physical, mental, and emotional health.
