Community Centers
Service Area: Chinatown and Sunset Park
The VNS Health Community Centers in Chinatown and Sunset Park are places for older adults to access health and social programs that will keep them connected, healthy, and safe at home and in their communities. Programs range from support groups to health screenings to exercise classes.

Chinatown NNORC
The community center in Chinatown is home to the VNS Health Chinatown Neighborhood Naturally Occurring Retirement Community (NNORC). The Chinatown NNORC helps Chinatown residents 60 years and older stay in their homes as they age. NNORC team members provide services such as case management, health education, and blood pressure screenings both in the community center and at home.

Hospice Outreach
Service Areas: Harlem and the Bronx
The VNS Health HOPE (Hospice Outreach Provider Education) Program is designed to improve access to hospice services among Black and Latino residents of Harlem and the Bronx. By working with neighborhood health care providers, faith leaders, and community groups, the HOPE Program helps people understand how hospice care can help them live more comfortably and have more time with the people they love.

Nurse-Family Partnership
Service Areas: The Bronx and Nassau County
The VNS Health Nurse-Family Partnership (NFP) is a free program for first-time parents that provides education and resources to support healthy pregnancies and babies. NFP nurses work with pregnant people as early in their pregnancies as possible continuing to their child’s second birthday to support their child’s physical, emotional, and social development.

Veterans Outreach
VNS Health Veterans Outreach connects veterans and their loved ones with a skilled veterans liaison, who will help them navigate the many benefits and services available through the Veterans Administration (VA).

VNS Health LGBTQ+ Outreach works with over 100 community organizations and health partners to provide education and resources to members of the LGBTQ+ community, as well as identity-specific training to medical professionals.
